Notice of Privacy Practices

What is HIPAA?

In 1996 the United States Congress passed a law called The Health Insurance Portability and Accountability Act, or HIPAA, which will go into effect on April 14, 2003. This law is designed to protect your privacy and to control how your health information is accessed and disclosed.

What is theĀ  Notice of Privacy Practices

The Notice of Privacy Practices is a document explaining how health information about you may be used and disclosed, and how you can access it. Starting on April 14, 2003, all returning students who have an appointment at Student Health Services will be given the Notice of Privacy Practices and will be asked to sign a statement that they have received it. (This is a one-time request, unless in the future the Notice is amended.) If you are an incoming student, the Notice of Privacy Practices and the acknowledgement form will be mailed to you as part of your SHS Pre-Matriculation Packet.
