Instructions for Submission for Existing Students
If you have already submitted your main immunization records and wish to add new records (such as flu shots, new immunizations (including COVID-19 vaccine) New lab work etc.
- Scan or take photos with your mobile phone of your immunization records (please make certain the copy is legible and right side up).
- Go to https://shs.upenn.edu Log in with your PennKey.
- Enter your COVID-19 Vaccine information and upload your record here by clicking the “Enter My COVID-19 Vaccine Information” button on the ‘Home’ page.
- For all other immunization records, go to “Forms and choose Immunizations Page” to enter the information and upload your records.
- When you are finished, click the button “Submit Final.”
